Plane flips over after pilot's switch blunder - ABC News

All Nippon Airways flight, with 177 passengers and crew on board, was heading from Okinawa to Tokyo on September 6 when it rolled more than 130 degrees to the left, leaving it flying almost belly-up.

The plane dropped 1,900 metres in just 30 seconds off the southern Shizuoka district.

The manoeuvre happened when the co-pilot, in trying to unlock the cockpit door for the captain who was returning from the toilet, mistook a command button for the cockpit door lock switch nearby.
